PLEASE READ ALL! This is a Re-DESIGN of my attached form. Attached you will find my form that I did it in Excel and must be kept in Excel. I attached my new company logo. The name of my school is JM SCHOOL OF REAL ESTATE, INC Please don't put the fax number, after "city" please add another line with "state". Please ad on the first part "how you hear about us " with choices with a little line space for people to make a check mark "phone call" "E-mail" "Past Student" "referred by ________" and "other". On the course selection part, please ad "Flexible hours course" and another course "Other". On the bottom part "payment Method" please replace it with " FOR SCHOOL USE ONLY". Please remove the 2 empty useless lines next to "check #". the credit card option should be a small little check mark to choose from the different options instead of the big lines, will be very nice if you ad a small picture of each credit card logo. Please remove the "check card" option". Please ad "course start date" and "finish date" with a line for me to write. Also ad "DBPR report date" with a line and :Data base Input: Yes or no check box. Lastly please ad "Instructor" with an option to choose from with a check box of "JM" or "LB".
Any question please let me know. This form will NOT be use online, it will be printed for students to fill in school, so I suggest don't do a square for people to write... instead use a line. thanks!

@contest holder please look into this matter as it is copied entry of my entry and its very discouraging and unethical, as the contest was not sealed this guy just stole my design and upload it with some tweaks he has copied my layout, colours, content positioning etc as you didn't provide any specific requirement about use of colour and layout its also indicate that he has copied my design, looking forward to hear from you, thanks.

  Hello timimalik, I wonder from which angle I have copied your design. The excel sheet was provided by the contest holder and I have designed the form according to the brief. Does that make it a copy of your work? From you verdict, it appears that the excel sheet and the brief was created by you. Isn't it? As per your report I have copied the layout, colors, content positioning etc. from you. Bro, I suppose you have missed out one thing -- the logo. Well, if taking the color from the logo and putting it on my design can make you say that I have copied your color, then why should the logo be left out? Isn't your report simply ridiculous? Think once again, why you have reported. If its just for the sake of winning the contest, here is my best of wishes for you.

Hey teamgraphic you just copy my design with some tweaks please man get a life you may win the contest but its ethically wrong and you should use your own creativity rather than stealing other design ~Good Luck
