Windows 10 Program to Copy / Paste Entire Address into Several Form Fields

I need someone to create a simple Windows 10 application (or Chrome add-on) which allows me to copy an entire address, then when I paste it into a form, the address fields should populate correctly into the form. For example, here is a sample address:
Company Name (optional)
First / Last
Address 1
Address 2 (optional)
City, State, Zip
Note that the company name and the address 2 field may or may not exist. Once this address is copied, I want to go to a separate website, and paste it in one single paste command. See attached screenshot what the form looks like. The country field will remain United States, and phone number can remain blank.
That's all. Very simple program.
Please note the copied "State" will be a two letter abbreviation. The program should select the correct state from the drop down list in the screenshot.
