Build me a simple webpage, with popup pulling data from database

I need a small web page created from the design provided in the PSD. In the middle of the page PSD there are fields (Select a sector, select a subsector, select an occupation, select a Job Role, Next) that I want to be drop down menus. The data required to populate these drops downs will come from the data in the excel documents I also provided. There are 10 excel documents and each include the fields that match the drop downs I need. This will need to be moved into a database format. I need for now the webpage to be built on your own servers that would then be packaged and delivered back to us after testing and validation of the correct data is linked into the webpage in the correct fields. After the user selects the dropdowns, and clicks on 'next', a popup will appear with the data they selected and then all the other fields that are now in the popup would be populated with additional information from the corresponding data cells in the excel documents. This would include data from the list of Tasks, Knowledge and Understanding, Core and Generic skills and Professional skills. We will need the data that is collected from the user's responses to the Yes and No questions to be summarized in a report format (Possibly PDF, or another HTML output) that would be provided by validating their phone number through SMS. The 'Report' design has not been created so any suggestions are welcome. Included in the scope of details provided is the PSD file for the design, a PDF showing the concept flow, and 10 excel files of data. Within the excel document is contains all the details that will need to be pulled into the web page structure. The PDF does provide a little more details to what I am looking to achieve.
