redesign my forum page and a list of other requirement

Add like Button, comment button, reply comment, share on other social media account and stuffs like this. At this page, a Notification emoji like normal social site but here doctors know a question is assigned, and their answers liked by users and users also know their question is answered. And forum users to know when their post is liked, commented or shared. Just like notification on the page. Anything just simple 2. At the end if a doctors answered questions user should rate if the answer was helpful or not and ask to thank or like doctor. 3. When a user post a question, on both pages the question should point the user location. Example see a 18 year old male ask ....... that is how it appear on site now but I want it this way.
