Write an iPhone application and Android application

I am working on a research proposal that would need an app developed for users to be able to answer a few questions daily or weekly as a "diary." More specifically, we are going to conduct a research project looking into smartphone apps as a means of collecting certain behaviors from participants. This will be a pilot study and around 30 participants will be recruited. Participants will be split into 3 groups. Group 1 will be required to answer diary questions on a daily basis Group 2 will be required to answer diary questions on a weekly basis Group 3 will be required to answer diary questions after certain behaviors are done (could be at anytime, e.g. each time they wash their hands) We are looking to use SurveyGizmo as a means of disseminating and collecting the survey data. We are looking for a developer to integrate this system into both an Android and iPhone App. Some additional info: -We want the participants to be able to have date/time of the data entry automatically done for us -We want participants to securely submit the data to us on daily basis (or whatever basis we end up deciding) -We want to be able to convert the app data into a spreadsheet such as Excel that we can analyze on our end
