Build a web-based GUI or input field program for an SQL database

I'm looking for someone who can build a web-based (in browser) GUI (securely password protected) with many input fields, as well as an SQL database to store the entries.

The database has the following requirements:

Able to store images (can be stored separately but must be noted in SQL database)

Able to handle multiple types of input (people, connections, notes, etc.)

Able to be searched via GUI for a number of parameters (all input fields individually or in combination + search for records that are missing ___ piece of data)

The input fields/GUI has the following requirements:

1. Able to enter records for people with the following fields:

Name

Occupation

History

Notes

Photo

Source

2. Able to enter that 2 different people are connected with the following fields:

Name 1

Name 2

Connection (brief)

Connection details

Source

Notes

3. Able to enter more than 2 connections in one go:

Name 1

Names 2, 3, 4, 5, 6, 7, 8, 9, 10 etc.

Connection (brief)

Connection details

Source

Notes

When entering connections (2 & 3), need to be able to choose from names already added, or add a new name, on the page for adding a connection.

In price of bid, please include 10 revisions + 6 months of support for any issues that arise.

Must be able to backup/download/save the database.

Database must be protected on website securely.

Must be able to eventually use the database to visualise the connections using a social network graphing tool at a later date.
