Need simple HTML5 to WordPress Implementation with small custom function

Hi, I have a HTML5 site coded with jQuery functions in it. I need someone who is super in WordPress to implement the design into WordPress. I'm implementing the majority of it, but there is a custom function that has options that need to be selected by the visitor and collected by WordPress, then emailed to an email address. These options need to be functional and dynamic. Please bid on just the functionality of those options and implementations of those pages, its functions. My budget is $150 for everything, this should include simple functions and 4 pages. If you read this entire description, enter KWD in your bid, Pages are very simple, two pages should just grab content from custom post types, like title, description and featured image...
