Simple address database

The script must be in php with mysql database. Simple script to store data in mysql database, search data and print it. The script will be used to send cards and ecards for birthdays, anniversary of the cities, new year and so on. - Multiuser, one admin and the admin only can add more users to access the script. Log of each login avaliable only to the admin. - Adding data thru HTML forms, with the following fields, with no mandatory field requeriment: Name; Email; Address (textarea, no need of zip or country field); City; State; Birthday (in format DD, MM, YYYY). - Ability to edit data. Only admin can delete a registry. - Ability to search for terms in ANY field being chosen at the moment of the search. In the results, all fields of the current entry should be shown and a special version of the search should show results ready to be printed on snail mail labels. - Mass emailer. - A button to search for birthdays. Once pressed, it shows the results of a selected month. For example, if now I search for April birthdays, all the April birthdays should be shown. - Ability to show results formatted to print on labels (for snail mail). - If possible, a way to import data from MS Word documents.
