I have designed the interface both in php and in Photoshop for this project. No database exist yet, that is where you come in, I need you to make all my prototypes described in the attached PDF’s come to life by way of mySQL Database. I’m using [login to view URL] for graphing and need you to be able to continue with this system to output user data.
The basics of this project are easy to understand. There are only two types of users. Coaches and Athletes. Coaches want to create groups of Athletes that will take a) 1 questionnaire each day and b) 1 form each day and based on the results show the athlete and coach via the website. I also need mobile versions created in HTML5 format that will work in any device.
The forms the athlete is filling out each day can be understood best in the following documents.

Once you get the basics of the two forms the athlete fills out each day. Lets start with the Administration area for this website in document Admin.PDF.
Once the administration area is up and running, then you will focus on two user types being Athletes and Coaches. I’ve broken those two documents into
