Dynamic PDF Generation in PHP based on given data

We are looking for someone to write a PHP script to dynamically generate a PDF with some fixed content and some content from a given dataset. The fixed content is in a PDF, so ideally your PHP script would take an existing PDF and add the necessary information to it. In the past we have used FPDF, but if you find another library is better, we are open to suggestions. Attached are 2 images: The fixed content PDF you will be modyfying as well as mockup of what the final product should look like after your script modifies it. The handwritten parts on the mockup is what you will be adding to the existing PDF based on data provided. The items listed in parentheses are the field names in the dataset. You will be provided access to an API to get the datasets you will need. You will also need to review a 2 page guideline on our coding standards to make sure your deliverable will work for our needs. A successful project will result in a php script that will download your modified PDF based on a given account id.
