I am a bit confused. What I understand is that we need to extract PDF form headers and create db structure for them. After that, enter data in the DB using a PHP (or any other technology) application. Finally, generate PDF form fill, which will fill PDF forms for each line in the DB and thus, filled forms for all records will be generated.
