How it got there. it might be outdated the or plugins or core. only host is capable to tell how its got there by reading logs of the activity by ip. is succuri not catching it ? I've cleaned out infections like this before. It's almost always php injection into a form, but i'll check all possible places bad code can be hidden. I've seen a few cases where attackers obtained credentials for a hosting account and put in many hidden backdoors.
