HTML templates for web (viewed as printable and to be converted to PDF) and email.

We nee the following two tasks: 1- Need to convert the attached PDF form "Registration [login to view URL]" to a printable HTML template. 2- Use the same content of the registration form to create an email template. Details: - The HTML template (from PDF form) will be used in generating dynamic forms and then convert them to PDF. The conversion to PDF is not part of the scope of this job, however, your HTML code should take that in consideration (if you have prior experience in converting HTML you will know what are these limitations). - Each PDF conversion library have limitation for what can be converted from HTML, you can test the current solution we have on this link: [login to view URL] just paste your html code there and test the current library limitations. - For PDF conversion, we are using FPDF library with this extension: [login to view URL] however, we are open for other options as long as it doesn't require installing binaries on Linux system (not allowed by hosting) or a monthly or per use charges. - Web (printable) template should be resembling the format of the attached "Registration Form" design, however, any improvement is welcomed. - Email template needs to be responsive template, we are currently using: [login to view URL], attached file: ([login to view URL]) is a sample template file. - Email template should be compatible with common email clients and web based email interfaces. - The design for the email template must not be an exact replica of the printable registration form, on the contrary, it needs to be reworked and we need an elegant and creative design for this template. - The design of the template should resemble the current branding and theme of our website: www.al-muyassar.ca. - Any improvement for the current template will be valued. Please note the following: - Please don't bid on this job unless if you have prior experience in HTML conversion to PDF using PHP. - We are not interested in a JS based PDF solution, we are strictly looking for a PHP based solution.
