Survey Generator

I want to create a small simple survey like this:

Please analyze the forms until the end.

Once the person posts their survey, other users are able to do the same survey (echos)

Instead of having a certain amount of inputs, I have a small dynamic form source you can use [or create your own] and I want to add a a title input. I would also want the 'last X amount of surveys created' saved anywhere but a database.

I attempted to create this script but the dynamic form killed me and I'm getting serious eye strains.
