My website is old and outdated. I want to redesign it and host it on http://www.squarespace.com

My old website was recently shut down because it’s outdated. Below is an old link of how the website looked. I don’t want you to copy the old look (maybe copy the colors) but I want you to make a new design on Squarespace for me.

This is a website for a medical clinic. Patients should be able to visit this site, see the information, fill out the patient registration form and access the questionnaire.

You should ONLY take the words and information from the front page, make a section for the patient registration form and highlight a link to see the questionnaire.

Honestly, I think WordPress will be the best option for your new website. WordPress powers a large portion of the web and google likes to give it a better ranking in the search results than sites built with other CMS's (wix, weebly, squarespace, etc)

Your provided link doesn't work properly. I have tried several times to load page from web archive but it was loading, loading and loading. Please attach pdf file in stead of link.
