Perl coder for very simple frontend

Scripting a very lightweight web-frontend which has following types a user needs to fill in:
- Person's name
- a selection field from where a expiration date can be selected
After supplying these fields and pressing submit the frontend inserts those fields into a MySQL database, including a self-generated password and a username and gives those back.
Instead of Perl you could also use Golang.
