296660 web driven email database

I need to be able to develope a database using mysql from a web page on my site. i would like to have aprox 6 fields and be able to change the fields for use on another page. I need the database to be compatible with excel so that each column is a field and each line is a record. I would also like to be notified by email when someone new submits their information.
