I have a [tax refund site][1] in the uk which requires the addition of a survey/questionnaire form which will offer forms to download based on the results of the questionnaire.
The site is written in php and is strict XHTML and needs to stay that way. No HTML tables are used. Some of the pages are dynamic.
It is hosted on a windows iis site hence the default.php.
I require a new page developing with some business logic and database storage.
I also need page level tracking and download tracking and automatic emailing all via php scripts … again this information is to be captured in a MySQL database.
Although my current focus is on site functionality, at some stage the site is going to get a design overhaul so this needs to be considered in the process of the development.
Content has been separated from presentation using very simple templates. This needs to remain so that design changes can be implemented system wide very easily. Plus I understand what is going on that way … although I wrote it, it is really just a prototype and I haven’t got the time to develop it. I need to focus on the business issues and build the agent network.
This is not necessarily a one off development. I think the site will grow and expand, so I will possibly be interested in an ongoing development / maintenance relationship.

## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
