Changes to existing opensource code

I need the below changes to the open source code here:

1)I want the dynamic fields to come from a database. I can give a sample DB schema to the shortlisted coder.

2)I want the submit button to call a WCF/Web Service from clientside and pass all the values to the service and save it.

3)The above code has to be in a separate project not in a MVC project. It will be a dll by itself which I can refer in my projects to use this functionality.

4)The forms has to be work even if I have more than one form in a page. ie, If I make user controls using the above form and include more than one usercontrol in a page this should work fine and has to only send data to the service for that particular usercontrol.

Coder should be able to flexible to accomadate small changes during the course of the project
