Complete List of Urgent Cares & Emergency Rooms across US

I am looking for someone who create a spreadsheet in Excel of ALL the Urgent Cares and Emergency Rooms in the United States. I am trying to create a complete directory for each state and its Urgent Cares and Emergency Rooms. The info needed for the directory will be the following: Name of facility Physician's Name (if applicable) Address of facility Suite number (if applicable) City State Zip Code Main Phone Number Fax Number Website of facility (if applicable) Hours Weekday (if provided) Hours Weekend (if provided) Obviously, there will be two different directories. One for Urgent Care and the other for Emergency Rooms. For most Emergency Rooms, the hospital information (name, address, phone #, etc) will be adequate. The directory will organized by States.
