iphone/ipad application - very simple "tap database"

very simple ipad/iphone tap database app - we have no problems buying and leveraging an existing itunes/android market available applications and providing us with instructions on how to set it up (eg we could buy Tap Forms HD and you would supply us with detailed instructions in how to set it up as per spec below) it is needed for rugby (football) statistics - there are several existing apps that do this by player but we only want to track at the overall team level as tracking by player is too complicated for games as they are occurring in the database we would be able to tailor and user-initiate the "elements" to keep statistics (by tapping on an iphone/ipad screen a green plus sign next to a red minus sign for getting rid of errors) on eg scrum won our feed scrum lost our feed scrum won their feed lineout won our feed lineout lost our feed tries scored tries conceded penalties conceded penalties awarded etc with master event (game) text fields data such as date opposition grade playing weather etc no problems with having an initial screen for setting the master data (eg date etc) then a second "data entry" screen for recording the stats themselves if the app can track the time either elapsed game time or system clock time for each event entered that would be a nice feature ability to email very simple reports of a dump of the stats - prettying up the stats would be done in excel etc and wouldnt be the gui for the elements/events would need to fit on one screen if easily able to be applied to android as well that would be great but not mandatory scribbled screenshot of what we're after attached
