Write an Android and iphone application

I am looking for an app for android and iPhone to help our workshop (vehicle repairer) sell work. At the moment we use report sheets and call the customer. The program we use is Carside by MAM. I want to have a program similar to this but there is many things missing from carside and there is no update in the future. I would like the report sheets to be a little more informative for the customer to help guide their decision and hopefully spend more with us. At the moment, the reports are made into a pdf from carside. I would like to keep the same format but Taylor it to our needs. Ideally I would like the report to show pictures, video link, additional summary box so we can see all items needed to be reported at the top of the page (items highlighted to corresponding colors), save to web space, text the customer a link, email the customer, email us, option to print. I would like to be able to completely customize each report like I can with carside for example one for servicing, MOT's, Winter check etc but from the app and have the templates viewable on all mobile devices or downloadable and stored on each device. Please take a look at carside, youtube has some good videos. Filling out the form should be simple for the technician with very few icons. I was thinking 6 boxes, Red, Amber, Green, Notes, Video, Camera. So a service check will be displayed on the screen asking what the wipers are like. If there hanging of the technician will click red, a notepad will pop up prompting the technician to explain why he has selected red. He will note down his findings and another box pops up asking if he would like to take a picture or record a video. After the page scrolls down to the next service check. In addition to the checklists, I also want a damage report system for when the cars come and go out. Im guessing I have not made this clear as I know what I want and have been using a similar product but please ask any questions. Thanks Lee
