I would like an Access Customer Database built from scratch (not from a wizard) to hold customer contact information, forms to enter in the information and reports to display customer visits, payment history, tutoring history, and payment plans, etc. Attached document includes: 1. A scope of the project 2. A confidentiality agreement that needs to be signed and returned within 24 hours of bid acceptance. 3. A Process Flow on how we will conduct business. 4. A list of all the reports and what they should look like when they print out. 5. A list of important notes/reminders. ***The coder is responsible for developing all tables, forms and reports. The reports are given below. From the reports the coder can make up his/her own field names for the tables. Please use consistency.

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased.
