Replace standard form upload with Multiupload solution on a PHP Form

**What**: The form is used to upload a various amount of PDF and SWF files to a web server, rename the file appropriately, and save information to a database. This creates a "lesson" in the database which is then displayed on a PHP page in a grid to users to download and view the various PDFs and SWFs.

Each PDF and SWF is tied to metadata that needs to be saved to the database. So it is very important to associate the correct PDF and SWF with the correct metadata. Right now this is done via sections on the form.

**Problem**: We have expanded the number of files that are uploaded at one time. There can easily be between 40-50 different PDFs and SWF files. This make the standard form upload process very touchy and it does not work most of the time. There are a lot of error time outs with no error codes to see what the problem is.

**Need**: Rewrite/edit this page to streamline the upload process and make it bullet proof. Ideally, we would like a multiuploader (html 5, no flash) that will show all the PDFs and SWFs that were selected in form added to a queue, upload each file individually, show progress for each file and progress overall, rename the files appropriately (using the same scheme used in the current upload form code), and write the corresponding metadata for each file to the database using the same database schema.
