Simple Mouseover Stars

I am looking for a star rating feature, like can be found here, [login to view URL] which I can add to pages in my website. As per the sample, the star colors will change as a person hovers or clicks on a star. Similar to the sample page, 1, 2 or 3 stars will need to pop-up a box that will display a form. (I will create the actual form myself, using RSForm Pro) 4 or 5 stars will pop-up a box that will show rating site options. This should be easily editable by me, as this star rating feature will be used on several different pages on my website, all requiring different rating sites and rating site landing pages. Similar to the sample, when a rating site option is chosen on the 4 or 5 star pop-up box, a new pop-up box will appear with general instructions on how to use the rating site, as well as a link to go to the rating site. I have attached a couple of star images to get us started. As mentioned, this star rating feature will be placed on several different pages on my website, which will show different review sites and different review site landing pages depending on the page. So these factors should be easily editable by me. Please feel free to ask any questions.
