Website Building and Coding Platform

I am looking for a website builder and coder. I am looking to semi-automate the ability to record healthcare notes for patient consultations and treatment sessions. The landing page will be a simple log in page. The home page will include a list of full customisable templates, which should be edit-able but also with the ability to add new templates. Each template should include text with the option to include macro buttons that add further text, multiselect or dropdown options. There should be an option to copy the entire document, ready to paste elsewhere.
